As veterinarians, we enjoy working with farriers to diagnose the cause of abnormalities or diseases of the hoof, like white line disease, quarter cracks, laminitis or abnormal growth patterns. Often, changes in the hoof reflect an underlying condition in the horse or lameness in another leg.       <image:caption>Gastric ulcers, also known as stomach ulcers are sores that form on the stomach lining. They are more common than you think and can range in severity from rarely showing outward clinical signs to severe cases that result in chronic colic. Gastric ulcers have been reported to affect up to 90 percent of racehorses and 60 percent of show horses. There is so much to know and learn about Equine Gastric Ulcer Syndrome (EGUS). Tune in to our Equiconnect Equine podcast episode about gastric ulcers with Dr. Sarah Shaw DVM, DACVIM where they discuss different types of ulcers, diagnostics, and treatments. Check out this real-life case study that takes us through the process of diagnosing an 11 year old mare with gastric ulcers and the successful treatment process. Often horses with ulcers present with a poor appetite, poor coat, recurrent colic, poor performance, cribbing, teeth grinding and sensitivity in the girth area however that is not always the case. Treatment protocols vary depending on the type of ulcer as well. Gastroscopy was key in the successful diagnosis of the types of gastric ulcers afflicting this horse and planning her treatment protocol.</image:caption>

      <image:caption>We are excited to have a new doctor soon-to-be-joining McKee-Pownall Equine Services for an internship. But what does that mean?! An intern is a graduated and fully licensed veterinarian who is seeking additional time after completing vet school to be mentored in a specific facet of the veterinary industry. In our case, an intern is seeking mentorship, experience, knowledge, and skill-building in equine practice. This is extremely important at a time when the equine industry is seeing a drastic decline in veterinarians. Our ability to provide a good experience for an intern will help ensure that the equine veterinary industry stays strong for years to come. Interns may be newly graduated veterinarians, or they may be experienced veterinarians who have decided to pursue specialized training in a specific area of the industry. So, what is an internship? An internship is a term used to describe the position an intern takes. Internships are often a year-long but can be longer depending on the needs of the intern and the practice. An internship is intended to be a year of mentorship where at the end the intern has gained significantly more confidence, experience, and skills in a specific area of veterinary medicine (like equine practice) than they would after a year of practice on their own. In return, equine practice (in general) gains a confident, experienced, and skilled veterinarian to join the workforce. Interns will typically spend the first few months working directly with a ‘senior’ veterinarian. As the intern gains experience, confidence, and skills, they do more and more on their own, until they are mostly working independently but always with a senior associate that they can call or request back up from. Almost every veterinarian at McKee-Pownall has come through an internship program and we are excited to be able to give back to our industry by offering our first-ever internship!</image:caption>

When do we recommend joint injections? - When joint disease/injury has been identified as a cause of poor performance or lameness by our veterinarian after a thorough exam, flexions, blocking and diagnostics - When a response to oral/injectable therapy has been suboptimal - When rapid, potent, localized effects are the goal - When the problem is isolated to specific joints What is Degenerative Joint Disease (DJD)? DJD can happen with repetitive stress or trauma that initiates inflammation, which typically begins in the soft tissue layers of the joint. The inflamed synovial membrane permits white blood cells to enter the joint. Enzymes and inflammatory mediators are then released into the synovial fluid. - If unchecked, chronic inflammation leads to degradation of the articular cartilage - Once severely damaged, cartilage cannot heal itself - Degenerative Joint Disease (DJD) eventually develops Signs of joint injury and Degenerative Joint Disease typically include: - Local swelling and heat - Altered stance and gaits - Pain on flexion and manipulation of joint - Poor performance - Change in attitude resists gate training or going to the track - Prolonged warm-up times - Good days and bad days (just like us!) At MPES, we offer in-depth lameness appointments across all of our service areas with our experienced team of veterinarians to get you answers and solutions for your equine partner. We also have two ship-in locations for horses that are not located in our service areas, as well as a standing MRI. What does a typical lameness appointment look like? The Exam This includes the history of your horse, a physical exam and watching your horse in motion. Flexions and Blocking Unless the horse is severely lame we will investigate further by performing flexion examinations and diagnostic nerve blocks. Flexions help us isolate a lameness to a region and diagnostic nerve blocks help us locate the lameness to a specific joint or structure. Diagnostics Once we have pinpointed a lameness in a specific area, we are then ready to take x-rays or perform an ultrasound to help us image specific structures and identify the cause of the lameness. Treatment We will then determine the best treatment option for your equine partner.</image:caption>

      <image:caption>Did you know that our Campbellville facility is home to a Hallmarq MRI? This system allows a scan to be performed on a standing sedated horse, which eliminates the risks of general anesthesia and hospitalization required with traditional lying-down systems. MRI is a diagnostic technique using a magnetic field to produce pictures of structures inside the body. MRI signals yield multiple images in the form of “slices” of the foot or distal limb in several different orientations. MRI provides images of bone, fluid, and soft tissue and can also help to differentiate between active and chronic lesions. This enables us to observe injuries in a way that cannot be seen with any other equipment. Benefits of the Standing MRI include: - High-quality diagnostic images of bone and soft tissue - Detection of lesions not visible on x-rays and ultrasound - Eliminate the risk of general anesthesia- - Outpatient procedure, no need for hospitalization - Major medical insurance often covers a portion of the cost - Accurate diagnosis allows for earlier, more effective targeted treatment plans and prognosis</image:caption>

      <image:caption>Navicular syndrome is the inflammation or degeneration of the navicular bone and its surrounding tissue. Navicular syndrome can be managed in horses, but not cured as it is chronic and degenerative. The most common effective treatments include corrective shoeing and non-steroidal anti-inflammatory drugs (NSAID) administration. This case example follows an 18-year-old QH gelding who was reluctant to move forward off the owner’s leg and his diagnosis process.</image:caption>

      <image:title>Lameness - The Exam</image:title>
      <image:caption>The Exam When we see your horse for a lameness examination we want to understand the whole horse. Even if we are looking for a lameness associated with one limb we want to investigate the other limbs and the body to see if there are reasons in other parts of your horse that may be contributing to the lameness. The first part of the exam is getting a history. Our vets will ask numerous questions to give us clues as we begin our investigation.  This is followed by a physical examination of the feet, limbs and body. While we have outstanding imaging equipment our fingers can tell us a lot as we palpate all over your horse as we look for areas of heat, swelling and pain.   We then follow this up by looking at your horse in motion. We look at your horse walking and trotting in a straight line to see if there are obvious signs of lameness, or irregularities in how their feet land. This is followed by lunging your horse in both directions at the walk, trot and canter on both hard and soft ground. Not only are we looking for a lameness, but we are also looking for any irregularities and stiffness in their stride or body, and how they carry their head. We will look at your horse being ridden when we want to see if a particular lameness is seen only under saddle. By the time we finish taking a history, perform a physical exam and see your horse in motion we are getting an idea of the lameness and are ready to move on to flexing the limbs and often diagnostic nerve blocks to help further guide us in our lameness examination.</image:caption>

      <image:title>Lameness - Flexions And Blocking</image:title>
      <image:caption>It is easy to assume we know where a lameness is located after finishing the first part of our lameness exam, but until we do further tests, we are just guessing. Making a diagnosis and treatment plan without further tests is often not in the best interest of you or your horse. When it comes to making a plan to treat your horse we risk treating the wrong area with the wrong protocol leading to further discomfort for your horse, wasted time and unnecessary expenses. Unless the horse is severely lame we will investigate further by performing flexion examinations and diagnostic nerve blocks. In the case of a severe lameness we will skip these further diagnostic tests since these could aggravate the lameness even more. Flexion exams focuses minor pressure on specific regions on a leg. It can help us identify if a lameness is in the carpus or in the fetlock of below in front, and the stifle, hock or fetlock and below in the hind limbs. The effect on a horse is very similar to us kneeling on the floor. Those of us with knee pain will be a bit creaky in one knee or both when we get up to stand. Flexions help us isolate a lameness to a region and diagnostic nerve blocks help us locate the lameness to a specific joint or structure. We use a numbing agent similar to that your dentist will use for a tooth filling. We begin at the foot region and work our way up. If the first block results in soundness we know the lameness is in the foot region. If the block doesn’t resolve the pain we move up the leg to target specific areas so we can tell if the lameness is between the foot and fetlock, the fetlock itself, between the carpus/hock and fetlock and so on. When we have pinpointed a lameness to a specific area we are then ready to take x-rays or perform an ultrasound to help us image specific structures and identify the cause of the lameness. If horses could talk we could skip many of these steps so in the best interest of the health of your horse we must use the techniques available to us to pinpoint a lameness.</image:caption>

The under tack pre-purchase option includes all of the standard in hand work, with an add-on of observing the horse under saddle and at all 3 gaits. Some conditions are only observed when the horse has the weight of the rider or with the addition of tack and rider. Other subtle conditions are exacerbated under tight turns, or when in a frame. This is particularly true for neck and back conditions. Any limb in question can be then reassessed and flexed under tack as needed. This adds another layer of information to the pre-purchase exam and gives you more information for your money.</image:caption>

We are now able to identify damage to these tissues, provide an explanation for many frustrating foot lameness cases, and suggest new treatments and shoeing techniques. MRI also allows evaluation of bone injury that is not visible on x-rays, such as fluid build-up and bruising, and early fractures before they become obvious cracks. The MRI system is best used when a lameness can be localized to a specific area, such as a particular foot or fetlock, but shows no abnormalities on digital x-ray or ultrasound.</image:caption>

Since graduating, Dr. Hernandez has worked with different species, from small animals to lions and zebras. However, her passion has always been equine medicine. Dr. Hernandez’ mother and grandfather are both Olympians and being raised with sport horses and knowing first-hand the influence they can have in the lives of their owners, she strongly believes in the importance of high quality preventive and sports medicine. She believes in the rider-horse-trainer-veterinarian partnership in order to get the most from horses and their riders at the discipline of their preference. Previously, Dr. Hernandez managed to balance her life as head veterinarian in a competitive equestrian team in Mexico and her life as a professional rider.</image:caption>

      <image:caption>Dr. Alejandra Garza obtained her veterinary degree from the Universidad Autónoma de Nuevo León (UANL) in Monterrey, México in 2007. Right after graduation, she started as an equine practitioner completing multiple externships and two rotational internships in private practice. The first internship was at La Silla Equine Hospital in Monterrey, México and the second in Alamo Pintado Equine Medical Center in Santa Barbara, California. In 2015, she completed a master’s program in Animal Science from the UANL with a research focus in equine musculoskeletal development and fracture repair of the equine hoof. In August 2015, she was granted a Fellowship in equine medicine in the University of California Davis where she obtained her diploma as a Junior Specialist in Equine Medicine and Field Service in 2017. In January 2018, she started a position as an associate professor in the UANL College of Veterinary Medicine teaching equine clinics, diagnostic imaging, and reproduction, while at the same time working in the Large Animal Teaching Hospital in the same school of veterinary medicine. In April 2020, Dr. Garza started her own equine only ambulatory private practice in Monterrey Mexico where the caseload was about 50% internal medicine (emergencies, neonatology, ophthalmology, respiratory and preventive medicine) and the other 50% in sports medicine and rehabilitation. Dr. Garza’s passion is equine medicine, practicing in this field for several years and having been trained with a high level of medical care in both private practice and academia with board-certified veterinarians has given her great experience. Dr. Garza enjoys providing supportive care to her patients and building strong relations with her clients.</image:caption>

      <image:caption>Dr Esther Millares started her professional career after graduating in 2012 in Madrid, Spain where she did an equine rotating internship at the Alfonso X El Sabio Veterinary Teaching Hospital. During this time, her interest in sport medicine and equine surgery increased and she decided to pursue a career in sports medicine. She then started a second internship at Paton &amp; Martin Veterinary Services in Vancouver, Canada, an equine clinic with a large orthopedic and lameness caseload. During this experience, she was afforded the opportunity to work with elite sport horses that suffered a range of orthopedic injuries and allowed her to refine her lameness skills. Afterwards, she completed a two-year Equine Sports Medicine Fellowship and a one-year Equine Field service Fellowship (Junior specialist) at the Veterinary Teaching Hospital of the University of California-Davis, USA. Dr. Millares then enrolled in an Equine Sports Medicine and Rehabilitation Residency program (ACVSMR Practice Experience), completed a certification in acupuncture by the Chi Institute in Florida, and achieved a kinesiotaping certification by Equi-Tape. During her time as a fellow and resident, she was able to develop some interest in research studies, which motivated her to pursue a Master of Sciences (MSc) at the University of Montreal (UdeM). Following graduation of the MSc, she was hired as an equine clinician at the veterinary teaching hospital (CHUV) of the University of Montreal and continues collaborating as a veterinary researcher at the Equine Asthma Research Laboratory of the UdeM. During her time as a veterinarian, she has taken many different opportunities to learn and become very proficient at the diagnosis of basic and complex lameness, regenerative medicine, diagnostic imaging, routine medicine procedures, and acupuncture among others.</image:caption>

Although this seems like a cheap way to address your horse's dental maintenance, however, In Ontario, it is illegal to perform dentistry on a horse without veterinary supervision. Certain individuals have been imprisoned for failing to respect this law. Follow this link to for more information on the CVO's postion on Lay Floaters. There are several issues that need to be clarified in this situation. Specifically, lay dentists do not: As well, lay dentists can not: Perform a thorough oral examination Use a speculum to visualize and give access to the rear teeth Use sedation to ease anxiety and smooth the process Give any pain control when it is needed Have specialized training to identify and safely treat dental abnormalities Prescribe drugs to treat oral infections Take x-rays of the jaw when indicated Perform extractions, or if they do, no sedation or nerve blocks are performed beforehand Offer insurance protection: if anything should go wrong, most horse insurance policies will be void because the procedure was not performed by a veterinarian.</image:caption>

She competed to advanced level in 3-day eventing throughout the United States prior to attending veterinary school. After graduating, she worked in New Jersey at a large equine referral hospital that provided surgical care, lameness, medicine, emergency, and ambulatory practice, followed by a year in equine practice in Alberta before returning to Ontario as a founding partner of McKee-Pownall Equine Services. She is certified in Veterinary Spinal Manipulation Therapy through the COAC. Melissa has served on the American Association of Equine Practitioners Scholarship, Racing, Education, and Ethics committees, and is an Equine representative for the Ontario Animal Health Network.       <image:caption>We have used acupuncture on a wide variety of cases and have had excellent success with quite a range of types of cases, such as: Sore backs and necks Navicular issues leading to foot pain Laminitis (acute and chronic) Tendon/ligament injuries Arthritis Extending the period between joint injections/reducing the number of injections needed Muscle atrophy/loss (Sweeny etc) Gastric ulcers Heaves/allergic lung conditions Anhidrosis or non-sweating Reproductive issues such as inability to conceive or maintain pregnancy In most conditions, three treatments are indicated.  Each treatment usually takes about an hour, and may involve electroacupuncture (gentle stimulation with an electrical current), plain needles or aqua-acupuncture (injecting into acupoints).       <image:caption>The first step, once you have decided to breed your mare, is to select a stallion and contact the stallion owners. You should find out which days the stallion is collected, how the semen is shipped and time frame for shipping. In addition, for international shipping, you must find out what import regulations exist. This information can be obtained from our vet offices, CFIA office, or online on the CFIA website. Next, your veterinarian needs to perform an initial assessment, including ultrasound examination, and other diagnostics depending on the mare's reproductive history. The initial ultrasound exam can provide information on when the mare is predicted to be in heat, and her potential timing of ovulation. Once the mare is in heat, she needs to be followed closely to predict the time of ovulation. For fresh cooled semen, the semen needs to be inseminated prior to the mare ovulating, and will generally remain viable within the uterus for up to 48 hours. For frozen semen, the mare needs to be inseminated within 4-6 hours of ovulation, and requires much more diligent monitoring of the mare through ultrasound. Ovulation is confirmed after insemination, and the mare is checked 14-16 days after ovulation for pregnancy.</image:caption>

      <image:caption>Stage One of Labour (30 Minutes to 4 hours) Your mare might begin to act colicky, lie down, sweat, look at her, flank, posture, lift her tail, urinate Uterine contractions are increasing and the foal is rotating position to enter the birth canal During this time you can clean your mare’s perineum and vulva with warm water and wrap the tail The chorioallantoic membrane ruptures (water breaks) and allantoic fluid leaks out Stage Two of Labour (20-30 Minutes) The foal begins to pass into the birth canal and there are intense uterine contractions The mare will often lie on her side or stand up/lie down again You should see a glistening white/grey sac (the amnion) begin to protrude from the vulva, The foal should be delivered with one front foot in front of the other, then the head If this continues for more than 20 minutes without any signs of a foal CALL THE VET Once the foals hips have passed through the pelvis the mare will often rest for 15-20 minutes As long as the amnion has ruptured, the foal is breathing normally and is lying in sternal they can be left undisturbed. The umbilical cord is best left to be ruptured naturally. o   NEVER cut the cord – this can lead to bleeding!   You can twist the cord, holding the foals umbilicus off and tearing on the mare side of the umbilicus if it appears to be having trouble tearing on its own. Stage Three of Labour (30 min- 3 hours) Expulsion of the placenta and fetal membranes o   The placenta can be tied up to itself to keep the mare from stepping on it and prematurely tearing Be sure to KEEP the placenta so your vet can make sure it has all been passed!! The handout below outlines the stages of parturition, common foaling emergencies and can be posted on your mare’s stall for easy reference during the heat of the moment! HANDOUT The Rule of 3 1)  The foal should stand within 1 hour 2) The foal should nurse vigorously within 2 hours 3) The mare should pass her placenta within 3 hours If any of these seem delayed call your veterinarian</image:caption>
